Introduction

Cardiovascular diseases are one of the most pressing issues in healthcare systems worldwide in the 21st century [2]. Historical global estimates cited in the source manuscript place cardiovascular diseases among the leading contributors to mortality worldwide. Because estimates vary by year, data source, case definition, and population, individual percentages should be interpreted in the context of the period from which they were derived.

Cardiovascular diseases, including ischemic heart disease [3], stroke [4], heart failure [5], peripheral artery disease [6], and other cardiovascular conditions [7], are leading causes of global mortality and make a substantial contribution to reduced quality of life [8, 9, 10]. The source manuscript cites historical estimates for 2012 of 17.5 million cardiovascular deaths, including more than 7 million deaths from ischemic heart disease and more than 6 million from stroke [4]. It also cites projected and historical WHO estimates indicating a continuing major cardiovascular burden [11].

Objective

To summarize the global and regional burden of circulatory system diseases, with particular attention to morbidity, mortality, selected country-level patterns, prevention, and health-system measures, including the experience of Kazakhstan.

Scientific novelty

The scientific novelty of this review lies in bringing together global, CIS and Kazakhstan data on circulatory system diseases within a single analytical framework that links epidemiological burden with prevention and health-system responses. The review highlights the need to interpret historical and cross-country estimates in relation to their time period and data source and identifies epidemiological monitoring, prevention, screening, service organization and continuity of care as interconnected components of cardiovascular disease control.

Materials and Methods

The article was prepared as a narrative analytical literature review with elements of secondary epidemiological data synthesis. The evidence base consists of the 20 sources listed in the manuscript, including international scientific publications on cardiovascular disease burden and risk factors, WHO strategic materials, publications from Kazakhstan and the CIS, official health statistics, and national program materials.

The source manuscript does not specify the bibliographic databases searched, the exact search dates, search strings, inclusion or exclusion criteria, a PRISMA flow diagram, or a formal assessment of methodological quality. Accordingly, the findings are presented as a narrative synthesis rather than a systematic review or meta-analysis. Quantitative indicators are retained as reported by the cited sources and should be interpreted in the context of their original country, population and observation period.

Results

Global and regional burden

The source manuscript reports that more than 1.5 billion people worldwide have arterial hypertension and cites WHO 2019 data indicating a substantial number of people with hypertension in Central and Eastern Europe. These figures illustrate the scale of elevated blood pressure as a major cardiovascular risk factor, although they refer to specific historical estimates and should not be interpreted as current universal prevalence values.

The reviewed material describes a high burden of cardiovascular disease in developed countries of Europe and North America, including substantial disability and mortality. It also presents historical sex-specific estimates and distributions of ischemic heart disease and stroke. The economic consequences are emphasized through the costs of treatment and prevention and the resulting pressure on national health budgets [12, 13, 14].

For the Republic of Belarus, the manuscript cites a cardiovascular disease prevalence of 28,920.9 per 100,000 population in 2017 and a 17.2% share in the overall morbidity structure. The source further notes that the introduction of high-technology diagnostics, treatment methods and effective medications may improve survival, but improved survival does not necessarily translate directly into better quality of life [15].

Among the CIS countries, circulatory system diseases are described as a leading cause of mortality. The manuscript cites figures of 52.7% of deaths in 2019 and 57.7% in 2020 for the CIS as a whole. These estimates reflect the reporting period of the cited material and should not be treated as current values without updated verification.

For Russia, the source manuscript reports a high proportion of deaths attributable to cardiovascular disease and identifies ischemic heart disease and stroke as leading causes within the cardiovascular mortality structure [16].

Kazakhstan

The source manuscript describes Kazakhstan as having a high standardized cardiovascular mortality burden in comparison with European countries and neighboring regions [17]. Because international comparisons depend strongly on the chosen year, standardization method and data source, the direction of the difference is more informative than an isolated numerical ranking.

According to official statistical data cited in the manuscript, selected cardiovascular indicators changed substantially over the period from 2004 to 2019. The source provides historical figures for overall indicators as well as myocardial infarction and stroke [18]. Differences in indicator definitions across years should be considered when interpreting these time trends.

Prevention and health-system response

Kazakhstan began implementing national healthcare programs in 2011, including the Salamatty Kazakhstan program and the subsequent Densaulyk program for 2016–2019. The source manuscript notes that these initiatives included updates to clinical protocols and regulatory documents, modernization of medical equipment, international professional training, reduction of equipment shortages in services for acute myocardial infarction, and development of stroke-control infrastructure [19].

To reduce cardiovascular morbidity and mortality, primary prevention measures are implemented at the state level. The source manuscript identifies health education in “Health Schools,” preventive programs, population screening for early detection, and the development of telemedicine and mobile healthcare. Early detection using modern diagnostic methods is presented as an important component of improving life expectancy and quality of life [20].

Discussion

The reviewed literature confirms that circulatory system diseases cannot be addressed solely through clinical treatment after complications occur. Population-level prevention, control of modifiable risk factors, early diagnosis, long-term treatment adherence and organized cardiovascular services must operate together. This interpretation is consistent with the emphasis in the source manuscript on prevention, screening, health education, technology and service modernization.

At the same time, the numerical evidence in the manuscript spans different countries and time periods. Some sources reflect the 1990s and early 2000s, while others concern the late 2010s and early 2020s. This heterogeneity limits direct quantitative comparisons. For a contemporary assessment, global and Kazakhstan indicators should be periodically updated using harmonized definitions and age-standardized measures.

Practical significance

The review supports several practical priorities for public health and healthcare management: sustained control of tobacco use, unhealthy diet, high blood pressure and other modifiable risk factors; accessible early detection and risk assessment; continuity of evidence-based treatment; monitoring of myocardial infarction and stroke outcomes; and evaluation of the effectiveness and equity of screening, telemedicine and cardiovascular service programs.

Limitations

This is a narrative review. The original manuscript does not provide a reproducible search strategy or formal assessment of the quality of included evidence. The references differ in study design, population, geographic setting and publication year, and some quantitative estimates are historical. Several cited sources address healthcare organization or prevention more broadly rather than epidemiology of circulatory system diseases specifically. Therefore, the review should not be interpreted as a pooled quantitative estimate of current global or Kazakhstan prevalence or mortality.

Future research

Future studies should update cardiovascular morbidity and mortality trends in Kazakhstan using recent age-standardized data, analyze regional inequalities, and evaluate the contribution of major modifiable risk factors. Particular attention should be given to the effectiveness and cost-effectiveness of prevention, screening, telemedicine, acute coronary and stroke pathways, and continuity of long-term care.

Conclusion

Despite advances in medicine, the persistent burden of cardiovascular diseases remains a major problem because these conditions can lead to death, disability and serious complications.

Disease prevention and early detection have a central role in preserving population health. Healthy lifestyle measures, reduction of modifiable risk factors, timely diagnosis and adherence to indicated treatment should be supported by effective cardiovascular services and reliable epidemiological monitoring. Shared responsibility between the health system and the population is important, but access to prevention and treatment must also be ensured at the system level.
